Agreement & Submission

Income verification shall be based on the applying family's federal income tax return for the most recently completed year, as filed with the IRS. The family must list all the dependents on the tax return, including the youth athlete that is seeking sponsorship. If there are separate parent/guardian tax returns within a household, then ALL TAX RETURNS MUST BE SUPPLIED. All tax returns must be hand-signed by the corresponding parent/guardian of the youth athlete. The Foundation may also request additional supporting documentation as it determines to be reasonable.

The Ballplayers Foundation is a 100% volunteer-run 501(c)(3) organization. Our goal is to support families with genuine financial need and help more young athletes stay in the game.
